Reports on investigations in England carried out by the Health Ombudsman. The first volume contains one-page summaries (epitomes) of key cases and briefer summaries of the rest. The contents page and an index classify cases by National Health Service sector and geographical area, and provide a quick reference to the matters investigated. The second part consists of the full, anonymised, texts of the reports. The cases cover many issues, including in-patient and out-patient hospital care, out-of-hours family health services, the work of NHS convenors, and the conduct of independent reviews.
